	Petition for the General Examination For students who have completed their course and language requirements and who have submitted their requisite seminar papers.
	Prospectus Approval To be submitted within six months of passing the General Examination.
	Application for Traveling Scholar Status For students who will be away from the Boston area, engaged primarily in their degree work.
	Petition for Extension of Time for an Incomplete Grade Students who received an incomplete grade for a course that was taken in the academic term prior to the most recently completed term must submit this form to be eligible to receive a grade upon completion of the work for that course. For example, if the current term is fall 2007, a student must submit and Extension form for an incomplete grade received in the fall 2006 term or any term before that.
	Application for Leave of Absence This form is for students whose time will be primarily devoted to activities other than degree work.
	Application for Readmission for Degree For students returning to EALC to submit their dissertations and receive the PhD degrees.
	Application for Academic Credit for Graduate Work Done Elsewhere Students who have completed courses at the Harvard Summer School or other institutions of higher learning must submit this form to the Registrar's Office in order to have these courses considered for transfer credit to GSAS.
	Completion of Degree Statement The department requests graduating PhDs to file a Completion of Degree form with the department. This asks for basic information on jobs and post-doctoral fellowships. The department uses this information to stay in touch with graduates and to furnish information both to GSAS and current students as to how EALC and HEAL PhDs fare in academia.
	Summer School Transcript Transfer Request This form is to request a transfer of a Harvard Summer School transcript to the GSAS Registrar's Office.
